When laptop resumes from sleep, a black screen is displayed for several minutes, sometimes a Blue Screen Mem dump video_tdr_failure occurs.
About 10% of time.

ITs now happening every single time my laptop comes out of sleep.

I have been trying to replicate same problem, I have tested HD 4000, HD 5500 and HD 4600 but I am not getting any Blue Screen.
I've followed your steps. I just made sure to update to the latest video drivers 4300
