Thank you for reporting the issue.
Please share your DxDiag report (press Windows* key + R key, type “dxdiag”, press OK and then click “Save All Information”). In order to attach files, make sure you select the "Advanced Editor" option in the top right of your reply window. You will then be presented with an attach file dialogue at the bottom of the reply.
Also include the steps to replicate the issue (where in the game it occurs) and cable brand/type, monitor brand/model.
