Which BIOS version do you have?

I'm having exactly the same probelm! If my Nuc (D54250WYK) looses HDMI connection (TV or AV-receiver shutdown) then my Nuc crashes immediately. I'm using the latest driver for Intel HD Graphics 5000.
I use the following adapters and components:
Intel® Centrino® Advanced-N 6235 (Blutooth, Wifi)
My Bios Version is 2.8. Im running Windows 7 64bit Version 6.1.7601 Service Pack 1 Build 7601. PLease find attached my system.nfo for further information.

I'm using the bios version that was installed by WYLPT10H.86A.0025.EB.EXE I believe it to be version 25.
I am using the built in IR receiver and a network tuner the "SiliconDust HDHomeRun PLUS HDTC-2US" but the issue appeared before setting up the IR receiver and network tuner.
I'm also having this problem. At first I thought it was my remote but I've stopped using it and I'm still having this problem. Checked CEC settings. Reflashed latest bios. Installed latest stable and beta video drivers all without any luck.
Beta 3540 driver (latest) has this problem - I wish Intel wouldn't leave it posted as "latest."
3496 driver does not seem to have such problem. Did you uninstall the 3540 driver before you tried installing 3496? That is what I did (re-installing without uninstall may not install older files over newer files).

I can confirm uninstalling the graphics driver then installing the driver from: Intel® Iris™ and HD Graphics Driver for Windows* 7/8/8.1 64-bit rather then then the NUC specific driver that is at Graphics: Intel® HD Graphics Driver for Intel® NUC has fixed my problem. The Intel HD Graphics Control Panel reports them both as "Driver Version: 10.18.10.3496" so not sure what the differences are.

I bought a Dr. HDMI from DHfury...problem solved with the original NUC HD5000 driver!
I have had the same issue with the following configuration:
Intel HD Graphics driver: 10.18.10.3540
Windows 8.1 x64
The NUC is running on the latest BIOS version (25).
I got this driver from the NUC downloads page.
Basically, whenever I would switch from PC mode on my receiver to some other mode, and then back to it, I'd get a BSOD.
After following Zaide_Chris' advice and downloading an older driver from Intel® Iris™ and HD Graphics Driver for Windows* 7/8/8.1 64-bit my problems were fixed.
So the driver I'm using now is 10.18.10.3496.
Same problem here with Intel 3410. I have a two display setup - TV and projector, connected to Onkyo 818 with two outputs. When I switch from TV to Projector, I get a blue screen of death.
I'll try the driver version mentioned above and report back. Also - I still get a ton of noise on my NUC when connected to the receiver. This is such a joke... how can this be possible?? It's over an HDMI connection, shouldn't it pass PCM DIGITAL audio?

Going back to the previous driver solved the blue screen problem.
But I still get a ton of noise and static on every remote button click and sometimes just because it feels like it... looks like it's a design problem, the machine is not grounded well. I have the device connected to a state-of-the-art power conditioner and still the same issue. Same issue on OpenELEC, windows or Ubuntu. Different power outlets (220v by the way) - same issue.
I am now confident this is a hardware issue and will be returning my Intel NUC. I'm very disappointed, really tried to make this working, but this noise is unacceptable for a machine that is designed to play music and movies on a sound system that cost a lot of money.
I am very sorry to report that same troubles are affecting NUC D54250WYKH and Intel HD Graphics 5000 drivers in Windows 7 x64.
When I turn off my monitor, I get a bluescreen and NUC freezes.
I had to uninstall Intel specific drivers and now using Windows Generic Graphic Driver the NUC seems to work.
Update: I am now trying the Intel Iris HD driver suggested above and it seems it is working.
Latest update: no way... blue screen again. The NUC seems to be more stable, but after a while a blue screen showed up.
I reverted to Windows Generic Driver.
