Been going through this problem for months. I have a HP Phoenix h9-110t desktop with Windows 8 64 bit. Can anyone here or from Intel help me solve the issue? To me it seems like a driver issue that needs updating? I am running the latest drivers that HP has provided. Instead of going into full details again and repeating. Its easier for me to post the direct link for you guys can see my information and take a look. I would greatly appreciate the help. No one at HP has been able to help me. Never ever had this problem with windows 7. Which the system originally came with it.
At this stage, it looks like you are missing some USB drivers, since this is win8, the drivers are not provided by Intel, and they are part of the operating system. If you are looking for an updated driver, you need to look for Windows Updates or through your computer manufacturer; they are familiar with the total configuration of your system.
I giot the same problem, randomly, on one or the other bus controller.
I'm fed up booting without mouse (there is absolutly no problem with linux, only windows 8).
My fix is a script, launched at boot start (windows task scheduler), and re-enabling the bus which is not started. Hope this helps:

It is important to say that when using Windows* 8 the USB 3.0 drivers are provided in the operating system manufacturer or the computer system manufacturer. It is also possible to get an updated driver through the windows updates.
In case that the same issue persists with these drivers, will be necessary to contact the computer system manufacturer or the operating system manufacturer to get the drivers for the controllers.
