Missing Tabs In Display Properties - INTEL GMA 4500M

Hi, I need help with my Display Properties. There are only 2 tabs left, I don't know where the rest went. Never seen this kind of issue before. I formatted my computer 2 times, but the problem isn't atil working.

Every time a reboot my computer it just disappears and I have to reinstall the Intel GMA 4500M.
