Addition: Video and audio output via HDMI.
I can confirm. Have the same problem on ASUS DZ77-V board, otherwise identical config as yours.
Seems interlaced handling is broken in this driver.
Have the same issue with config:
CPU: Intel i3-3225 (HD4000)
MB: Biostar H61MH
Driver version: 220.127.116.11.2875 (x64)
When I switch DXVA2 mode to DXVA in registry, all works fine but with higher CPU usage.
It seems that it's 100% driver issue.
