Have you tried booting up into Safe Mode and checked if this behavior replicates? If under Safe Mode you are able to see both monitors it means that you are having issues with the Operating System or drivers since this mode does not load the Operating System drivers. If it was the case that you get positive results under Safe Mode, we would then recommend you updating to the latest graphic driver version. If you already have the latest, you could rollback to the previous and test; if that does not work, you can update once more to the latest version available and test again. Also, did you make any system changes such as a BIOS/driver updates? And, could you please let us know which is the graphics driver version installed on your system?

Hi Karla, Thank you for providing detail guidance below. I followed your guidance and found the following. I would very much appreciate if you have any other ideas for how I may debug the system and get the connectivity to Extended Monitorback:
(1) When I go into SafeMode, I do see both screens operational. One is a 55" Samsung TV connected via the DVI/VGA connection. Another is a Sharp 50" tv connected via HDMI cable. Both screens duplicate the exact same thing.
(2) When I boot without safe mode, only the 55" Samsung connected via DVI/VGA works once the windows login screen is displayed.
(3) I tried the routine with downgrading and upgrading the graphics driver. Nothing helps. It does not work either way.
(4) I did upgrade the BIOS, in the hopes that it would help resolve the issue.
(5) I do understand your comment that there must be something wrong at the OS level. I just can not figure out what. I also notice that Microsoft SilverLight crashes whenever I try to use it.
(5) I also upgraded all drivers. Exact details of the system as detected and reported by Intel tools are:

This issue most likely is due to an Operating System corruption, our last recommendation would be performing a fresh installation and downloading the latest drivers available on our site instead of loading the ones from the installation disk.
The drivers have been available for a while now, therefore, we can assure the driver properly works. This leaves as the only option, the Operating System.
