iaStor.sys causing Blue Screen

I am running SRT in enhanced mode on my brand new Gigabyte Z68XP-UD3 board with a 2600K processor.  My accelerated drive is a 2TB Hitachi (port 1) and my cache is a 60GB OCZ Agility 3 (port 0) running on a SATA 3 port.  I have no other internal hard drives.

Lately, I have noticed that iaStor seems to forget which drive is being accelerated.  It will report that the cache drive is operating just fine, but it can't find the drive to be accelerated.  Of course, the accelerated drive has my OS on it, so the very fact that I can get to iaStor means my accelerated drive is accessible.  Also, in the right hand column, iaStor shows my 2TB drive.  Usually I am able to remedy this by resetting my cache drive and assigning it to the 2TB again, but the problem reoccurs on reboot or wake from sleep.

This morning, on wake from sleep, iaStor caused a BSOD (see details below), and after a reboot, shows no cache drive (the SSD is not even visible in the right hand column, it shows port 0 empty).

I chose the Z68 chipset specifically because I wanted to run SRT, and I bought the SSD specifically with that in mind.  I am very disappointed in the performance (or lack thereof, since SRT hasn't been able to survive a single reboot), and would really appreciate any advice on how to resolve this.  Also, let me know if I have left out any important information.  I thought I included everything, but I know sometimes seemingly minor details can be important.

    Hi there!

Let's make sure you are using latest version of the Rapid Storage software version 10.8, according to software release notes, it fixes fail caching after resume from S4, and this might help:

    Hey Alan,

Thanks for the tip.  I upgraded about a week ago, and turned off sleep mode on the computer to simplify things.  I just had another blue screen immediately preceded by an update from iaStor saying something about acceleration being disabled (it blue screened before I could read it).  I have pasted dmp information below.  Also, now that I have booted up, iaStor reports the following: The accelerated disk or volume associated with the cache device is inaccessible, and the overall performance is no longer optimized.  Please reconnect the accelerated disk or volume, or click Accelerate for more troubleshooting options.
